The BG+ Classic Standard and BG+ Classic Elite are medical program packages with group life insurance benefit designed specifically for small businesses with 5 to 19 employees

BG+ is a combination of comprehensive health care plan and group life insurance. It gives you access to comprehensive health care benefits with out-patient care, emergency treatment, preventive health care, and in-patient services. The insurance component includes MBL (dreaded disease/ICU coverage); and for the principal members, a group life insurance, Accidental Death & Dismemberment (AD&D), and Total Permanent Disability Income Benefit (TPDIB) of 60 months.

BG+ Classic Elite Program provides access to Cardinal Santos Medical Center, Makati Medical Center, St. Lukes Medical Center (Quezon City), The Medical City, EXCEPT Asian Hospital and Medical Center and St. Luke's Medical Center-Global City.

Group Insurance Benefits:

1.  Group Yearly Renewable Term Life (GYRT)
      -  Provides 24-hour coverage on and off the job (Suicide is not  covered)
      -  No geographic limits

2.  Accidental Death & Dismemberment (AD&D)
      -  Provides 24-hour coverage on and off the job
      -  Pays an additional benefit or percentage thereof if the employee dies or loses his limbs as a result of an
         accident

3.  Total & Permanent Disability Income (TPDIB)
      -  Pays a monthly income in the event of total and permanent disability.
      -  Total Permanent Disability may be due to bodily injury or illness.
      -  A monthly pension payable in 60 months


Coverage Guidelines:
>    Group must have an employer-employee relationship
>    Group must have at least 5 to 19 regular employees
>    100% of eligible employees must enroll whether  contributory or non-contributory. For contributory group, the minimum contribution rate from employer is 50% of membership fees
>    Dependents may be covered for as long as participation rate should be 100%. Different rates shall apply and there would be no group insurance coverage.
>    Account must be of standard occupational & health risk
>    No waiver of pre-existing conditions for new accounts.
>    Customization of benefits is not allowed.
>    Baby Group Plus enrollees shall be treated as group enrollees as such the group underwriting guidelines for 20-99 employees shall apply.
>    Applicants 51 years old and above are required to undergo a Medical Examination at PhilCare clinics.
>    All employees are required to fill-out PhilCare application form
>    All members of the same rank should enroll under the same plan.
>    Modes of Payment: Annual, Semi-Annual, Quarterly or Monthly
>    No franchising  of account is required.
>    Rates for the comprehensive plan package and Group Insurance are integrated.
>    Rates are age rated.
>    If the  number of enrolled members  falls below the required minimum which is 5 employees, the company will still be required  to pay for the membership fees  corresponding  to  the shortfall  in  the number of members.  Therefore,  no  refund  of  membership fees  for cancelled members will  be  allowed  below  the  minimum requirement of five actively  enrolled  members.
>    Changing of plan within the contract year may only be allowed if it is due to promotion.
>    The 31-day grace period for payment  shall be applied.
